Add knowledge artifacts

Upload one or many PDFs, images (OCR), Markdown, or plaintext files. Nexus processes them in parallel batches (bounded by UPLOAD_PARALLEL_CONCURRENCY), chunks with overlap, embeds, and stores vectors in Postgres.
